When to Keep Your Child at Home
PLEASE KEEP THEM HOME IF THEY HAVE A FEVER, VOMITING, DIARRHEA. They are to be symptom free for 24 hours without the aid of medication prior to returning to school.
If your child wakes up in the morning with a headache, stomach ache, or generally not feeling well, please keep him/ her home. Medications
- Please be sure your child’s medical information is current on your child’s FACTS profile.
- Medications will be accepted on an individual basis and administered only as prescribed by a physician.
- All medications should be sent to school office in the original container, with the prescription label attached, including the physician’s directions for dispensing the medication. All medicines must have a MEDICINE ADMINISTRATION FORM with them.
- Non-prescription (over the counter) medications will be accepted on an individual basis as provided by the parent or legal guardian provided a completed authorization to give medicine form (included in registration packet) is on file in the office. Prolonged use of non-prescription medication is discouraged. Physician or health care provider approval is required for use of non-prescription medication. The medication is to be sent in the original container. Examples include cough drops, Tylenol/aspirin, lotions, creams, etc.
Health Records
Kentucky Department of Health requires health records be maintained for all students.
Immunizations
Effective with the 2018-19 school year (July 1, 2018), the following new immunization requirements for elementary school students (Grades K-8) will be in effect:
- All students should have two (2) doses of Hepatitis A vaccine. Parents should start the process of vaccination now because the minimum time between the doses is six (6) months.
- There is a new format for Kentucky Immunization Certificates, and all students are required to obtain and submit the updated certificate to schools.
All parents should take the following steps:
- Consult with your child’s healthcare provider to ensure that these vaccines have been given to your child, or make an appointment for your child to receive the vaccine(s).
- Obtain an updated Kentucky Immunization Certificate from your provider.
- Turn in a copy of that Kentucky Immunization Certificate to the school office.

Eye Exam
Archdiocese of Louisville Catholic elementary schools require proof of a vision examination by an optometrist or ophthalmologist. Submit proof to the school no later than January 1st of the FIRST year that a three (3), four (4), five (5), or six (6) year-old child is enrolled. Vision examination information may be reported on the Kentucky/Archdiocese of Louisville Eye Examination Form for School Entry.
Physicals
We want all of our students to maintain regular physical exams. Physical exams are required for all incoming sixth-grade students before the school year begins.
Communicable Disease
A letter will be issued as needed to either grade level or school-wide for any communicable disease or ailment (Chicken Pox, lice, COVID, Hand-Foot-Mouth, etc.) confirmed by a doctor.
